Below is a list of several ways to apply knowledge of the content. This will improve learning and demonstrate understanding as well as provide valuable scholarly contributions for others. The Application Rubric contains the criteria for evaluating this and other work. See also the guide for presenting work. Work presented outside of UniCommons.com must be referenced (e.g. web link, embed) and explained on the group page so that all work is documented under the user's account.
==ESSENTIAL APPLICATIONS==
Physical Activity Readiness Questionnaire (PAR-Q)
Complete the PAR-Q form before doing any physical activity for this class. If you answered "YES" to any of the questions, provide a detailed explanation to your instructor. DO NOT attempt the fitness test until you have clearance from your doctor and you have consulted the instructor. Please include other concerns you may have that are not addressed in this questionnaire.
Information Sharing
Demonstrate diverse ways of sharing information. Some examples are provided below under "Recommended Applications."
Scholarly Sources
Show proficiency finding, recognizing, and referencing appropriate scholarly information. This shall include ample evidence of properly citing and referencing a variety of different resources in APA format (e.g. journals, websites, media, and other electronic sources).
Networking Platforms
Demonstrate the ability interact in scholarly online networking venues including at least the following:
- Blackboard
- Group page at UniCommons.com
- One other
Fitness Test
Pass the following fitness test battery at the 65th percentile:
- 1 cardiovascular test: 1.5 Mile Run recommended.
- 2 strength tests (for different muscle groups): Sit-up and Push-up test recommended.
- 1 flexibility test: Sit-and-reach test recommended.
Download and read the "EOU Fitness Test Battery." The protocols for the selected tests must be specifically followed for the results to be accepted. Note that a common mistake is not performing the 1.5 Run Test on a standard track (i.e. do not use a treadmill or road course). Only approved tests with appropriate norms will be acceptable. Submit the results on form for Fitness Test Scores.
In addition to submitting the scores using the above form, the tests will need to be verified in ONE of the following ways:
- Proctor: Arrange for a physical activity and health professional to proctor and verify the test. The proctor must have an appropriate certification (e.g. physical education teacher with current teaching certification, personal trainer with a license from a nationally recognized organization). Before arranging the test, the proctor and their qualifications must be approved by the class instructor. In any case, the student must arrange for someone that is qualified to administer the test and maintain integrity of the results. The proctor must verify the test results, age, and identity (show photo ID) by completing and directly submitting an Activity Verification Form. Give them the following link for completing the form
http://www.unicommons.com/forms/activity-verification-form - Video: Using an approved protocol, video tape the test. The video tape must continuously show the subject and provide close-ups of any measurement scores (without interruption immediately following the performance). The video will need to provide enough evidence to demonstrate integrity of results. If the 1.0 mile walk test is used, subjects must have on a heart rate monitor in order to display heart rate on the video immediately after completing the test. See the guide for working with video. Make sure the video is compressed, and then uploaded for private viewing by the instructor on the fitness test score entry form.
- In Person at EOU: Arrange for an Eastern Oregon University physical activity and health professor to administer the test for you in person, on campus. If an arrangement cannot be made, one of the other options must be used. Or, arrange a test at another university kinesiology department, which will likely charge a fee.
Half of the total points will be awarded for completing the fitness tests according to protocol and the other half of the points will awarded for achieving the 65th percentile or higher, based on established norms.
Personal Success Planning
Develop a plan for personal and career success. This may include some of the following:
- Personal mission and goals
- Graduate school plan
- Degree plan
- Professional portfolio development
- Documenting progress with a vita

Document Sharing
Demonstrate sharing of documents in one or more of the following standard formats:

Demonstrate how to share media in one or more of the following ways:
- Video embed
- Presentation embed
- Audio (Podcasts)
- Picture slideshow or thumbnails
Online Databases
Using the online databases that are available from the EOU Library website, search and find the following:
- A peer-reviewed research article published in 2007 related to physical activity and health.
- An article published in Research Quarterly for Exercise and Sport within the last year. Include the details about the article (author, title, date, issue, etc) and the abstract.
- A practical (non-research) article from a good source in a physical activity and health field.
- Submit the web address to the online form for requesting an article from interlibrary loan.
For each, include the details about the article (author, title, date, issue, etc) and a 2-3 sentence paraphrase of the article.
Website Hunt
Your challenge is to search for a professional website related to the field you are interested in pursuing. The site must include content that is academic in nature--something that will help you with your career. You must provide a working link to the website in the appropriate threads started under the forum. There are 4 stipulations though:
The link must be to the main page of the site, not a sub page within a site.
You cannot use a website link that is covered in class (on the online class notes).
You cannot use a site that has already been posted by someone else. Because of this, there will be some incentive to get the assignment done early; otherwise, you will have to check all the previous links to make sure you don't repeat one.
Graduate School
Determine a graduate degree you are interested in. Then, find universities that offer the degree and narrow your search to 2 top schools that you are interested in attending. For each of the 2 universities, search their website to find and submit the following information:
- Name of degree (e.g. MS in Exercise Physiology)
- Due date for application
- List of entrance requirements/standards
- List of application items/guidelines
Mission & Goals
To be an effective professional, you must have a clear purpose and direction in your own life. Your assignment is to develop a personal mission statement and goals. A mission statement is a concise statement about your purpose in life. For example, "Become strong mentally, physically, socially, and spiritually." Another approach you can use for a mission statement is a short them like, "Do your best." Goals are much more specific but should support the mission statement. Goals should also be specific and measurable. For example, "I will graduate by Summer 2008." Another goal may be, "I will attain a job in the fitness field within the next month." Your goals should be organized into at least four short-term goals (under 1 year) and at least four long-term goals (1-5 years). Each goal should include an anticipated completion time.
Your mission and goals are constantly evolving so they should be updated often. As you review your mission and goals, you will be able to measure your past success and develop a sense of direction for the future and security in the present.
Degree Plan
Compile and submit a degree plan that includes all classes from the Freshman year to graduation. Use the following guidelines:
- At the top, list the exact name of the degree you are seeking along with the concentration(s).
- For general education classes, specifically indicate which area (e.g. Natural World) and how many credits that class will be. You are not required to list the exact class.
- Everything should be organized according to term (e.g. Fall 2007).
- Any classes that are already completed should be identified.
